    • Overview

      Pharmacists and nurses possess pivotal roles regarding heart failure management. Numerous studies have demonstrated improve clinical outcomes for heart failure patients when an intensive medication reconciliation and discharge medication review were conducted to monitor for dosing appropriateness, omissions, duplications, and drug interactions by the pharmacist.  Likewise, multiple studies have demonstrated that individuals with heart failure are unable to describe their medication regimen, do not recognize worsening symptoms, and are unaware of how much sodium they should be ingesting. These gaps in knowledge hinder patients’ ability to engage in effective self-care. Nurses and pharmacists, then, play a vital role in closing these gaps.  This session seeks to arm pharmacists and nurses with the information needed to make meaningful clinical interventions for patients with heart failure.  Various pharmacologic and non-pharmacologic strategies are discussed within the context of understanding heart failure on fundamental terms.
