    • Overview

      This module focuses on identifying patients who are eligible for a MTM service and subsequently scheduling those patients to receive their respective services. First, the learner will explore how patients are identified. Although this process may vary slightly across MTM service billing platforms, there is a common theme: the Medicare Part D health plan sponsors utilize an algorithm that classifies patients for the appropriate MTM service based on the patient’s profile information in accordance with CMS eligibility requirements. Once patients have been identified within the MTM service billing platform they can be scheduled for services by pharmacy personnel. Next, appropriate steps for preparing to reach out to a patient to schedule services will be discussed. It is important that the pharmacy technician review the patient’s profile to adequately prepare the patient for the review. The pharmacy technician will also need to ensure the patient understands the purpose of the MTM service. Finally, the learner will explore best practices and requirements for the space that will be used for the MTM service appointment.
