    • Overview

      Shared decision making for medication initiation has shown to improve adherence and outcomes in chronic disease management. Shared decision models transform the conversation between clinician and patient and this program will give specific examples of how pharmacists can use shared decision-making techniques in achieving guideline directed medical therapies. The 5-step process developed by the Agency for Healthcare Research and Quality on the shared decision-making process will be reviewed.
