    • Overview

The purpose of this CE presentation is to highlight the recent updates to the Targeted Medication Safety Best Practices for Hospitals guideline document published by the Institute for Safe Medication Practices (ISMP) in February 2024 and provide guidance on the role that pharmacy technicians may have in upholding these guideline updates. Recent updates include information on safeguarding strategies against incorrect-route errors in administration of tranexamic acid, the implementation of strategies to prevent medication errors during transitions of care, and mitigation of errors associated with vaccine administration in both the inpatient and associated outpatient settings. This program aims provide a deeper understanding to the various components of hospital workflow that may lead to these medication errors and elucidate how pharmacy technicians can make an impact.
