    • Overview

Polypharmacy (taking 5 or more medications regularly) is growing in prevalence. This may result in an increased risk of drug interactions and adverse effects. The stakes are particularly high in patients living with a serious illness. Participants in this session will learn about how to select and discontinue medications that are specific to the patient's goals of care, particularly in light of their serious illness.
