    • Overview

      The final module in this certification will begin by briefly discussing how the pharmacist creates the Medication Action Plan (MAP) by compiling information and data gathered throughout the review process and the creation of intervention claims. Next, a discussion on claim submission for Comprehensive Medication Reviews and Targeted Medication Reviews will discuss the information required for each type of claim submission. The pharmacy technician can, again, play a vital role in communicating the pharmacist’s recommendations to the prescriber, documenting the prescriber’s response, and submitting claims. The importance of having a tracking system throughout the MTM service process will be highlighted. Finally, the learner will explore potential metrics that can be used to measure the success of a pharmacy’s MTM program.
