    • Overview

      This session seeks to explore the role of the pharmacist in the management of patients with heart failure. Approximately 25% of hospitalized heart failure patients are readmitted within the first 30 days, often due to medication nonadherence, inadequate understanding of the disease and/or treatment, and the inability to recognize key signs of exacerbation. By categorizing medications as mortality and morbidity reducing agents, this session seeks to assist the pharmacist in exploring medications commonly used in the treatment of heart failure. Notable clinical trials are used as the foundation for best management practices for heart failure patients.
